Information to gather

  • Patient name, date of birth, state, and current contact information.
  • Facility names, provider names, departments, dates, and discharge dates.
  • What went wrong, when symptoms appeared, and when the injury was discovered.
  • Current diagnosis, disability, later treatment, bills, and work loss.
  • Complete records requested, received, and still missing.

Privacy and consent note

Do not send sensitive medical details to a website unless you understand who receives them and how they will be used. If a lawyer, referral service, or advertiser is added later, the page should identify that relationship before asking for contact details.

This static page does not collect forms. Use it as a preparation checklist before contacting a licensed attorney.

Questions for the first attorney call

  • What deadline could apply in my state?
  • What records should I request before the review?
  • What type of medical expert would likely be needed?
  • What injury or added treatment is tied to the alleged error?
  • How do fees, costs, and expert expenses work in writing?
