Use it to organize facts and documents before an attorney review. It does not determine whether malpractice occurred and should not delay medical care or urgent legal advice about a deadline.
State Deadline Review tasks
- ☐ Record date of each treatment or alleged error.
- ☐ Record when the injury was discovered or should have been investigated.
- ☐ List later diagnosis and corrective-treatment dates.
- ☐ Identify whether the patient was a minor.
- ☐ Identify any public hospital or government defendant.
- ☐ Ask whether notice, certificate, affidavit, or pre-suit screening rules apply.
- ☐ Ask about statutes of limitation and repose separately.
- ☐ Have a licensed attorney calculate the deadline; do not rely on a generic chart.

Keep sensitive information private
Do not place Social Security numbers, account passwords, full medical-record sets, or other unnecessary sensitive data into a worksheet that might be emailed or shared casually. A lawyer can tell you what documents are appropriate to send.
