Pharmacy-error review begins with the prescription and what was dispensed
Prescribing and pharmacy errors are related but not identical. Compare the original prescription or electronic order with the pharmacy label, product, quantity, instructions, refill history, counseling documentation, and the patient's later medical records.
Records to preserve
- Prescription or electronic order
- Pharmacy dispensing history and label
- Photographs of packaging or tablets when relevant
- Insurance claim record or receipt identifying the dispensed drug
- Communications with the pharmacy or prescriber
- Medical treatment for the alleged adverse event
Questions that distinguish pharmacy from prescribing issues
- Did the pharmacy dispense what the prescriber ordered?
- Was the strength, formulation, quantity, or instruction different?
- Was an interaction or allergy alert documented?
- What injury followed and how was it medically evaluated?
