Communications can fill gaps between formal visits

Portal messages, phone logs, texts, and callback notes may show when symptoms were reported, what advice was given, whether follow-up was requested, and when the care team became aware of a change.

Preserve the communication trail

  • Full portal thread rather than a single message
  • Call date/time and number or office
  • Voicemail or callback documentation
  • Attachments or photographs sent with a message
  • Subsequent appointment, order, or advice linked to the communication

Distinguish memory from documented communication

If a patient remembers a call that is not in the chart, make a separate dated note describing the memory. Do not alter screenshots or label an undocumented memory as a facility record.
