A clot can occur despite appropriate care. A review may consider documented risk factors, symptoms suggesting deep-vein thrombosis or pulmonary embolism, preventive measures, diagnostic testing, and the response once a clot was suspected.

Records to collect

  • Admission and postoperative risk assessments
  • Mobility and prevention orders
  • Nursing notes documenting leg, chest, breathing, or vital-sign changes
  • Ultrasound, CT, or other diagnostic reports if obtained
  • Anticoagulation records and later treatment

Questions for expert review

  • What risk factors were known?
  • When were symptoms first documented?
  • What diagnostic steps were taken?
  • What injury resulted from the clot or alleged delay?
