Identify the level and timing of the spinal cord injury

A spinal cord injury file may involve trauma, surgery, compression, infection, bleeding, or delayed recognition. Review the neurologic examination before the alleged event, imaging, procedure records, and the first documentation of new weakness, sensory loss, or bowel/bladder dysfunction.

Core records

  • Neurologic examinations
  • Spine MRI/CT reports and images when available
  • Operative and anesthesia records if surgery is involved
  • Postoperative monitoring and escalation notes
  • Rehabilitation and functional assessments

Causation questions

  • What condition threatened the cord before treatment?
  • When did neurologic function change?
  • What intervention is alleged to have been delayed or performed improperly?
  • What permanent deficit is medically attributable to the challenged care?
