Identify which limbs or functions were affected, when weakness or loss of movement began, and what underlying condition, procedure, spinal/brain event, or other mechanism is alleged to have caused the change.

Causation and function
Qualified experts may need to separate paralysis caused by the underlying disease or injury from additional impairment allegedly caused by delayed or improper medical care. Document mobility, self-care, bowel/bladder function, work, and future-care needs from actual records.
