Trace the medication from order to administration
An overdose-related review should identify the medication, ordered dose, route, timing, administration record, monitoring, and any later intervention. The analysis may involve prescribing, dispensing, administration, or monitoring rather than a single provider.
Documents that can reconstruct the event
- Medication orders and reconciliation records
- Medication administration record (MAR)
- Pharmacy dispensing records
- Weight, renal/hepatic information, or other dosing inputs documented in the chart
- Monitoring, laboratory, rapid-response, or transfer records after the event
Questions that help identify responsibility
- Was the alleged problem in the order, dispensing, administration, or monitoring?
- Were there duplicate medications or transcription issues?
- What symptoms or objective findings followed?
- What alternative explanation for the injury should an expert evaluate?
